We have not and don’t plan to for a while.
Not yet. Mainly because, other than when traveling or going places for work, I don't tend to eat out a lot. And I haven't been doing much of that travel or going places for work in recent times. And I guess I've also been keeping the freezer and fridge stocked a lot since COVID started, so I don't really have those "man, not much here, so why not go out for something..." days right now. Closest for me is ordering out, but that's always take-out.
We’ve done indoor once (our country club), outdoor several times. We’ll continue to do both regularly.
I just golfed at the old Longaberger golf course and got a couple beers with the guy I was paired with after our round at the bar. It felt normal. I liked it.

That's how I felt last night. Buddy has a campsite near Loudonville/Mohican area and after they allowed those to open in late May, he and his wife have been working on their site pretty regularly. Invited me out last night for dinner and booze and it felt so damn great to have a night like that. Felt like life was becoming "real" again. At least until I woke up this morning. Apparently it felt so great that we were doing shots until 4 a.m. Definitely a Danny Glover in Lethal Weapon "getting too old for this shit" moment!

To Mad Dogs in Marysville since a buddy owns it. It opened less than a week before they shut down the state.
Only thing that would really keep me from going is if places are busier with everyone wanting to go, and/or less capacity.
