Has anyone not heard of this expression? I used it today and a co-worker asked me wtf I meant by that. I assumed it was pretty standard, but maybe it's local? I dunno.

More generational than geographical, I think. Heard it growing up, but don't recall hearing it since.

I guess it's fallen out of use so much, none of the youngsters learn to suss out its meaning, or at least know how and when to use it in conversation and writing to add a little spice to their communication. And I am officially old.

I've never looked it up where it came from, but from the context in which I've always heard or read it, I assumed it had to do with being insulted so much, it shows on your face.
