If you are talking worldwide rivalries and worldwide interest to the rivalry. I have little doubt its Barcelona vs Real Madrid.
Rivalry is beyond football. Barcelona is in a part of Spain that wants to be its own country. Its like a cival war of politics. That and more people watch that game than any other sporting event each year outside the Super Bowl and CL final.

If that's the criteria then India-Pakistan cricket might be the tops.
