Scary is pretty subjective. I thought Aliens was "scary" as in intense. The scariest movie where "scary" is disturbing is 120 days of Sodom based on the Marquis De Sade book. It is an Italian film and it is so disturbing that most people cannot watch it all the way through.
Cannibal holocaust is another one that is pretty distrubing.
