It's happened once in the Women's tournament, which is really surprising because there is a much bigger talent disparity and less parity in the women's game than in men's basketball.

I think it is an issue with the seeding. Gonzaga won a ton of games, but they don't look like they should have been a 1 (they earned the 1 with their regular season performance). And then Southern didnt look like any 16 I've ever seen

Meh. Relatively, maybe. If you were grading basketball ability on a 1-100 scale, the difference between the worst and best men might be 40 to 100. The difference between the worst and best women would be more like 3 to 15. Gonzaga did precisely what a good portion of us here thought they would do. Mark Few looks like he was just robbed at gunpoint. Few couldn't find Atlanta if he were staring at a map.
