What type of picture has to be marked "NSFW" and please explain why it needs to be marked. What is everyones opinion?

Yes they all should be except the first one. It should be marked Not Safe For Life.

Is the rule of "NSFW" mean that any person in a bikini, regardless of age, body, or sexiness has to be marked?
How about this one? I am just trying to get the ruling on what is "NSFW" since I posted a pic of my nephews ex girlfriend and everyone went crazy because they like to post on the OC while using a computer at work.

1. Go to tools
2. Click on internet options
3. Click on content
4. Uncheck load images or load images automatically
5. Click ok
