Not being active is #1. A distant #2 is allowing personal bias to influence their moderating decisions. A closer #3 is no one respects a vast majority of the moderating staff. I don't think I'd love the bias for purpose of moderating, so I could see that being annoying.
What is the plum line for respect of a moderator? What are the tenets of good forum leadership? As for the trolling ... okay, I agree with that as well, unless they're trolling a troll to get him/her to leave. Hell, I did that with the pizza spammer guy.

Active moderation is the same as active posting. If you aren't posting, you aren't moderating; fair or not.
Good forum leadership is setting an example for your fellow community and generating discussion for said community. When's the last time a moderator created a thread that generating a discussion? Justin does it sometimes, but he's too busy trying to get negs working for everyone.

Leadership and moderating aren't necessarily the same thing.
