Shaun Alexander. The guy went from one of the best RB in the league to nothing.
This list of baseball players goes on and on. Bonds, Richard Hidalgo, Matt Morris, Preston Wilson, Melvin Mora, Bill Mueller, Eric Gagne, Vinny Castilla, Erubiel Durazo, Mark Loretta, Chad Cordero all off the top of my head.

Terrelle Davis. The guy rushed for 2000 yards and won two super bowls with the Broncos. He was an amazing back, but his knees couldn't take it soon after the last super bowl.
