Saw a match Sat where one Wrestler head-butted another and then proceeded to swear at the other Wrestler - does OHSAA get involved?

Depends on the situation ? Usually warning buy ref , then points taken away but like I said depends on the situation of the head butt

Did the ref take any official action, like a DQ ? If so, I believe there would be an OHSAA mandated suspension, like a fighting DQ in basketball.
