My one recommendation is to try to keep it as short and simple as possible. As you pointed out, response rate might be an issue. And anything long is more likely to be ignored. I would keep Keep it to 4-5 Questions with a multiple choice. Open ended answers can sometimes be seen as too much work (or maybe leave a space for "other" and describe").

Stuff like:

1) Why did you stop posting on OC?
a) Lack of time
b) Over enforcement by Moderators
c) Under enforcement by Moderators
d) Aggressive behavior by other posters
e) other; explain (leave space)

What improvements would bring you back to the site?
a) Less rules regarding behavior
b) More rules regarding behavior
c) Enforcement of current rules
d) Change in moderators
e) Bans of posters
f) other; explain

Yeah, I definitely want to do simple multiple choice questions. No more than a handful or so.

Multiple choice questions with a short text box afterward to explain in general detail would be good. You could offer those text boxes after each question, or just one at the end.

As I mentioned in the other thread, it needs to somehow be done where each email has a unique link that can only complete the survey once, so that it's not just CoA and l_t going to surveymonkey and filling it out 50 times each.

Well, they wouldn't get the email. I'd run a query based on people that haven't posted in, say, the last 30 days that have more than X number of posts total.

You can almost guess a good majority of them already.

Rock, SQ, dynasty, coyotes, etc...posters

Wayne Co dbags (ringz, beast, etc), SOS, capone, few others from hs forums....moderation

I think of the surveys you get back, it will be about 50/50.
